This website, occuchart.com, is operated by Sigma Sage Consulting LLC, a Georgia limited liability company based in Marietta, Georgia ("Sigma Sage," "we," "us," or "our"). OccuChart is our occupational-health documentation and reporting product.

By accessing or using this website, you agree to these Terms of Use. If you do not agree, please do not use the site.

2. What this website is for

This site provides general information about OccuChart and a way to contact us about it. It is an informational and marketing site. Using it does not create a customer relationship, a contract for services, or any obligation on our part to provide the platform to you.

The OccuChart application itself is hosted separately at app.occuchart.com and is accessible only to authorized users of organizations that license it.

3. No medical advice

Nothing on this website is medical advice, clinical guidance, or a substitute for the judgment of a qualified healthcare professional. OccuChart is documentation and reporting software. It does not diagnose, treat, or recommend treatment, and we do not provide healthcare services. If you have a medical concern, contact a licensed clinician. In an emergency, call 911 or your local emergency number.

4. Acceptable use

You agree not to:

• use the site for any unlawful purpose, or in violation of these terms;
• attempt to gain unauthorized access to the site, the OccuChart application, or any related system, account, or network;
• probe, scan, or test the vulnerability of our systems without our prior written permission;
• interfere with or disrupt the site, including by introducing malware or by placing undue load on it;
• scrape, harvest, or systematically collect content or contact information from the site; or
• misrepresent your identity or affiliation when contacting us.

We welcome good-faith security research. If you believe you have found a vulnerability, please email sigma@occuchart.com before disclosing it publicly.

5. Do not send us confidential or patient information

Do not submit protected health information (PHI), patient records, or sensitive personal information through this website or through the contact email addresses listed on it. Those channels are not intended or secured for that purpose. If you need to discuss patient data, contact us first and we will arrange an appropriate channel under the applicable agreement.

6. Intellectual property

The site and its contents — including text, graphics, layout, screenshots, the OccuChart name and logo, and the software itself — are owned by Sigma Sage Consulting LLC or its licensors and are protected by intellectual property laws. You may view and print pages for your own internal evaluation. You may not copy, republish, modify, or use our content or marks commercially without our prior written permission.

7. Third-party links

The site may link to third-party websites or services we do not control. We provide those links for convenience only and are not responsible for their content, practices, or availability.

8. Availability and changes

We may change, suspend, or discontinue any part of this website at any time without notice. We may also update these Terms of Use; the revised version takes effect when posted, and the effective date above will change. Your continued use of the site after that constitutes acceptance.

9. Disclaimer

This website is provided on an "as is" and "as available" basis, without warranties of any kind, whether express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the site will be uninterrupted, error-free, or free of harmful components, or that the information on it is complete or current.

Product descriptions on this site are general summaries. Specific functionality, service levels, and commitments are defined only in a signed agreement.

10. Limitation of liability

To the fullest extent permitted by law, Sigma Sage Consulting LLC will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of profits, revenue, or data, arising out of your use of or inability to use this website. Our total liability relating to this website will not exceed one hundred U.S. dollars (US$100).

This section limits liability for this website only. Liability relating to the OccuChart platform is addressed in the signed Software & Services Agreement.

11. Governing law and venue

These Terms of Use are governed by the laws of the State of Georgia, without regard to its conflict-of-laws rules. Any dispute arising out of them will be brought exclusively in the state or federal courts located in Cobb County, Georgia, and you consent to personal jurisdiction there.
